I am looking for some kind soul who has a copy of the "published"
(in which Chapter V has been revised since submission) but out-of-print
thesis "Recursive Domains, Indexed Category Theory and Polymorphism" by Paul
Taylor right close by and would mail me a copy of (missing) pages 123-126
and 139-140.  It's copyrighted August, 1986.  (I do not wish to trouble again
my distant original source who had to do a librarial search to find it and
then gave his time to make me a copy of this thesis.)
