After posting a request for help on the above, several people offered
solutions, but there seems to be a trick.  In order to correctly get
HFS on the disk you have to hold the option key down for the whole
formatting/verifying/directory creating procedure.  Thanks to 
Russell Hobbie <hobbie@mailbox.mail.umn.edu> for pointing out this
important detail, and to all other who sent me mail.
