The WELL is reached long distance (out of the SF Bay Area) vis
CompuServe's Packet Network, or CPN.  You dial the local CPN
telephone number (get the number from Compuserve Information,
an 800 phone call -- watch out!), then type   WELL   at the
prompt, "Host Name:"  (Oh yes, you must hit a return on first
getting the CPN connect, to bring up the "Host Name:" prompt.)
Then log in.  I don't know if you can establish an account 
this way.  I don't see why not.

CPN charges the whopping side of my WELL bills.  I think it's
$4.50 an hour now.  Still a lot cheaper than a long distance
phone call, but you must factor this in with your account.
