One good source is John David Miller's _An OPEN LOOK at UNIX_, M&T Books, 1990. This has a good set of samples for individual widget usage, plus a chapter that details ICCCM conventions and the OPEN LOOK extensions of these to interface with file manager and workspace manager. It is not a comprehensive manual, but it does provide a good overview and a jump-start on OLIT programming. One version of the book comes with a 5.25" 1.2 Meg cpio disk targeted at 386 UNIX boxes, with all the program examples and the source of a nice little utility library Miller uses in the book.
