Making a system crash proof and being bug free are two different concepts. Making a system crash proof sometimes requires considerable overhead, such as always checking processes to see if a deadlock has occurred. If the *designers* of the system decide that deadlock is so rare that the cost of detecting or avoiding it is not worth it, then they won't put those features in. (No, we don't use this system for air traffic control 8-)
