I had similar problems in November building TeX on a DECstation 3100 under Ultrix 4.0. Various pieces of text in math mode simply disappeared. I sent mail to the maintainers of web2c, and got a reply from Karl Berry telling me that the version I had ftp'd from labrea.stanford.edu (version 5.0c) was obsolete. His suggestion was to "Get 5.8a from ics.uci.edu [128.195.1.1], in the directory TeX." Apparently the problem is due to a missing cast somewhere. I haven't done this --- I had already succeeded in building a working TeX on a machine running Ultrix 3.1 (with optimization), but Pierre MacKay sent me a message saying that he had built a working version with optimization on a beta-test version of 4.1.
