I have an application for which an error diffusion type dither isn't
appropriate, but an ordered dither would be perfect.  However, I can't
see how to do an ordered dither with an arbitrary palette, only with
a uniform one which wouldn't work in my case.  Is it possible?

p.s. Actually I can see one method, but it would require eight (at least)
inverse palettes, which would take too much memory.
