We have a 4D/60T with a drive that's unreadable.  We need to reformat it with
"fx", but the volume header is scrambled and we need to rewrite the parameters
(tracks/cyl, etc.)  The drive has a metal tag on it that says "Micropolis M/N
1558" which I assume is the model number.

Does anyone know what numbers I should feed into "fx"?
