Has anyone out there in X world created a widget that
will take in a bunch of text with spots where users
fill in blanks with info?  Kind of like a hypertext
thing, expect that instead of buttons imbedded in the
text, you have editable text fields instead?

I'd appreciate E-Mail if you have one..
