In the cases I know about where an exchange name was two words, the two letter prefix was taken from the beginning of the FIRST word. On the east coast, I have heard of MUrray Hill in New York City, MOunt Vernon in the nearby town in Westchester by that name, and CHestnut Hill in Philadelphia.
