I have done some programming for the Paradise VGA + in the 640X400X256
mode. ALL the information required to use this mode was provided by Paradise.
The one problem I found was a bug in the _Setpalette function provided with
QuickC, I had to write my own with in-line assembly language.  If you get
one of the books available on VGA programming it will give you the info
you need to write that part. The bug was that _Setpalette worked fine in
standard VGA but not in SVGA.( Paradise claimed it was Microsoft's bug
but who knows??)
