Have you yet considered the possibility of a particular piece of software that
might cause the problem??  Perhaps one of those mouse accelerator facilities.
I have found some programs that work well most of the time, but occasionally do
some nasty damage to my drives.  Also, you say you've swapped everything out.
Does this include both the drive and the controller?
