Folks, I know this has been discussed at one point in the dim past, but I don't
remember the solution.

I've got an Apple CD-ROM player, chained off my IIfx with a Third Wave TEAC
tape and a Hammer 300 FMF (in order, Mac--CD(id3)--Tape(id5)--HD(id6),
terminated with the black terminator.  The disk and tape work peachy, but
9 times out of 10, when I insert a CD, it won't mount (I get the <eject>/
<init> dialog after a few minutes of lock up).  I've reinstalled the software
multiple times.
