Our problem turned out to be (of all things) the VGA cards! We couldn't get a thing on the second monitor. We mucked around a bit and found that if we used a Video7 VGA card, lo and behold, the mono screen worked with CVW. Makes no sense, completely crazy, but that is what happened. Its either related to video BIOS, or perhaps the MONO card is actually one of those (pseudo) smart cards that is actually MDA/CGA and is stuffing the system up a little at POST time.
