Is there any UNIX-based, Public Domain software available (with Source code)
for Software Problem/Response Reporting ?

The software should support the following:

	(i) Define a Database for each Software Product
	(ii) Define a Group of Software Developers for each Product; these
	     people should get automatically informed when a new problem is
	     reported.
	(iii) Each Problem can be uniquely identified
	(iv) Anybody can report a problem
	(v) When a problem is fixed, the Reporter gets automatically informed
	    about it
	(vi) Generate Summary of Problems Reported and Fixed
	 ...........
