I find it easy to understand, although the speed is on the slow side. It is also difficult for software to control it cleanly, so it squeals and makes noises some. Finally, it is an external unit which means that the text to speech algorithm is on-board, so I don't know about converting to Italian. I know that Dolphin systems has units with forgein language modules, but I don't know if Italian is one. ...
