I have done that -- I used a standard serial card as a basis, changed the crystal to a 2MHz (instead of 1.8...MHz), and replaced the RS-232 driver & receiver section with a current loop receiver driver, including an opto-isolator ... a friend told me he took a standard serial card, changed the crystal, cut a few tracks, added a few links, and put the opto into the RS-232 driver socket.
