Since this has come up a couple of times, I thought I should respond.

Yes, I do have 'sort of' a version of the reference in TeX.  This is done
via an internal HP formatting language front end that is translated into
TeX.  It is very depedent on some HPTeX macros and fonts.  I haven't bothered
to go through and make the thing 'pure' TeX.  If you are really interested,
I could be coerced into sending a printed copy.  (I have already gotten in
touch with Brian.)

If anybody is really keen on this and wants to do the conversion, let me know.
