Hi,
  you should not use format unit (0x04) just to initialize the floppy
drive. This will instead try to format the floppy diskette !
  mode select, rezero unit
should suffice.

P.S. check your mode select parameters, that is where an illegal
field would most likely occur. Or, a non-zero "init" byte in the
format instruction
