I have two questions that aren't in the FAQ. Sorry of this is a
repeat.

What general mechanisms are used to associate user specified icons
with programs?  If a user doesn't like a default icon, how does the
user create an icon and associate it with a unique program?

I am familiar with some of the techniques, but there doesn't seem to
be any conventions.

I would like to generate a summary of the different techniques.
