I have recently gotten overwhelmed for requests for this. I guess no one knew about the bus on time being too long for overlapped floppy I/O. If you will drop me a line, I will Email it to you. And that goes for anyone else who is using a floppy tape in conjunction with the AHA-154x adapter.
