Well, I may be able to find you a clue.  B-)

The easiest thing to do is ftp pub/gnu/etc/DISTRIB from prep.gnu.ai.mit.edu.
This contains a copy of the "current" order form.

I put quotes around the word "current", because we are in the process of
putting together a new distribution.  If possible, you'd be better off to wait
till July 1st so you can get the very latest distribution.
