The first such that I have heard of is when a young Japanese airman, taking off from IJN Taiho during the Battle of the Phillipine Sea, spotted incoming torpedo tracks (from USS Albacore, as I recall.) He dove on one of the torpedoes, crashed into it and blew it up, along with himself. Certainly an active defence :-) Taiho was hit by other torpedoes though, and later blew up due to poor damage control.
