I'm fairly certain that I'm using the sling correctly since the sling at 
the upper swivel is smooth below my left hand (I am right handed)

Do you have the loop placed above or below the biceps?

Thanks, I am aware of the Creedmore sling and I may have to buy it if I can't
use the issue sling high above the biceps.
