On Crystal Failure:
1) It is possible to overdrive a crystal and crack it.
2) It is also possible to drive a crystal at an unintended harmonic,
depending on your circuit.
3) See Horowitz & Hill (The Art of Electronics) for a good crystal
oscillator.
