You have exactly described a problem that I experienced a while back after installing an STB Powergraph ERGO-VGA card. I finally came to the conclusion that it must be a hardware problem, most likely with the video card. I decided to reseat all socketed IC chips on the VGA card. When I pulled out one of the DRAMs I found three of its pins skewed outward. Not out of the socket, just outward. I straightened them out and haven't seen the problem since. After reading your message I'm not so sure this was the cause of my problem. Your VGA card may use the same chip set as mine and perhaps it's a problem with that chipset.
