bad-block handling arrives
Modified mkfs, dump, dumpdir, and restor for bad-block handling.  Mkfs
now takes a bad-block list in its arguments, and puts those blocks in
inode #1.  Dump, dumpdir, and restor will never touch or manipulate
that inode (in fact, any inode < ROOTINO).

This brings us to roughly where we were before the great disaster.
