man(8) fixups
Fixed the manual-page maintenance stuff to run properly in the presence
of a restrictive PATH setting (such as it gets from rc via cron now)
and to update the table-of-contents properly when a manual page is
deleted or renamed (still doesn't get rid of obsolete preformatted pages,
though).
