While we're considering injuries in the chemical labs, how about:

distillation rigs that explode when they run dry (due to peroxides in the
solvent)

flasks with circular scores in the glass on the bottom - the bottom drops
out when you tap it too hard (e.g. when breaking up a chunk of solid you
want to dissolve)

and that famous lab caveat: Hot glass looks exactly the same as cold glass!
