Thanks to all who responded with useful suggestions.  The problem
turned out to have nothing to do with the LHDH itself at all, but
was a bad DEC Unibus terminator board (bad capacitor).  It was
the suggestion to look closely at where the Unibus plugged into
the LHDH which enabled us to spot a cracked capacitor.  

By the way, ACC seems to have fixed the problem with the shorted
traces.  There was no hint of that possibility on our boards.
