I, for one, don't mind asking the man out first.  I don't consider myself a
forward female, but I do like taking the initiative.  Sometimes, if I waited
for the guy I'm interested in to ask me out (whether he's shy or unaware) I'd
be to old to go out.

Yes, the first date should be over lunch or maybe a company function.  It's 
easiest to get to know somebody in casual surroundings where you both feel
comfortable.  As a rule though, if I'm doing the asking, then I pick up the
tab.  And later, if all works out and we begin dating regularly then there is
no reason why I can't pay occasionally or reciprocate with a home cooked meal.
I think dating is a 50/50 deal and should be treated as such, especially where
$$$ is concerned.

Only occasionally have I initiated the date, but I can't ever remember being
turned down (knock-on-wood).  Most have seemed pleased and even a little
relieved.
