The .NH instruction has been changed to allow two extra arguments to specify
the numbering of levels one and two.  So, .NH 1 4 will start the numbering at
4. and .NH 2 4 3 will start the numbering at 4.3.  This feature is intended
for when a section of a document is printed by itself.	(This is intended to
replace the .SE NHL instruction mentioned a few days ago.)

A new instruction has been added:  .LI will draw a horizontal line the width
of the page; an optional argument is used to specify a length (eg. .LI 3i to
produce a 3 inch line).
