A recent news item at the University of Waterloo:

>  Due to unforseen hardware difficulties, we have successfully

There had been a three step plan:
  (1) install the new disks
  (2) test new disks with the old processor
  (3) install the new processor.
But, when they got to stage (2), it was discovered that a cooling fan on
the old processor decided to act more like an arc lamp than a fan, so they
skipped that step and put in the new processor (with a new fan of course).
