	... at least some stores want a phone number and address in case you
	forget your card or another package you were carrying ...

I couldn't say if that's WHY they do it, but I have had a card returned
to me by a store because they had gotten my phone number off the bill (well,
the principal reason was I was silly enough to leave it behind, but the
phone number permitted them to return it...).
