No one would deny that huge bureaucracies are inefficient and, more important,
unresponsive.  The challenge is to achieve smallness within the bigness.
Have local bodies handle much of the details of implementing policies, with
the federal government spending its time on national issues and national-scale
planning.   National Health Insurance could just as well be called Universal
Health Coverage -- the important thing is not its national character, but its
universality.
