Kirk did report it and had it classified to prevent curiosity seekers from visiting just to look. 2. Just because Chekov was not on ST that season does not mean he wasn't on the Enterprise. In fact, it is stated that he was basically a rookie and had indeed seen Khan. 3. It is also stated that the only survey records that were available came from an old survey vessel, and that such records were often in error.
