The DH does not have to be the same throughout the game. You can pinch hit for him or pinch run for him. The only exception is that the pitcher cannot be a sub for him. Once a pitcher comes to bat,* there cannot be a DH. Also, if the DH of record becomes a fielder, the DH again disappears and the pitcher must hit. *A pinch hitter who is on the roster as a pitcher is NOT a pitcher for this rule. Only those who pitch in the game count, i.e. the one who ended the previous half inning or who starts the next one.
