Their supervisors are listening to them, and they will get repremanded (or fired!) for not saying what they have been told to say, whether you care to hear it or not. McDonald's pays their counter help about $0.13 an hour, and with that kind of incentive, it's not likely that the counter folks will come up with a more creative reply to a "no thanks," or the like. The consumer's solution to this problem is quite simple: the kitchen stove, or even the microwave, will not ask you if you would care to try some new "Fried McLizard Guts," and will save you considerable expense, not to mention a reduced consumption of cholesterol, calories, and pulverized chicken feathers.
