The book is called "Dr. Who.  The Making of a Television Series" by
Alan Road, Puffin Books, Penguin Books Ltd, Harmondsworth Middlesex,
England, 1983.  ISBN 0 14 03.1687 6.

Penguin has a US office at 40 West 23rd Street, New York, NY 10010.

It is available in hardback and paperback.  I got the paperback one
for $5.95.
