For a start, it doesn't handle Upper-lower case distinctions across source files. One example (this is a plug) is UNITY/VMS from HCR, with which I happen to be associated. The compiler and linker are from BSD 4.1, with an additional system V compiler in the works (we are waiting for Release 2).
