San Diego Lines: 17 <> Thieves only get the double, triple, etc. damage from behind when they surprise people. So, if a thief were to get multiple attacks, the first attack would be at a +4, and do multiple times normal damage. However, the second (third, and fourth) attack would be at his normal to-hit, since the creature he is attacking presumably knows he's there and is defending against his attacks.
