It seems to me that a compiler's difficulty in dealing with a segmented address space is just that: the compiler's difficulty. I have worked with many compilers that are able to produce large-model code for an 86/88 processor, and one of them is a C compiler (ex from Mark Williams, now sold by Intel). Admittedly, it is a true pain in the neck to deal with individual data structures larger than 64K in size on the 8088, but in my experience that doesn't happen *too* often (only when writing the interesting stuff, it seems).
