The main reason for this that I have experienced is that the site is not in our site table. Seems to me that there is an obvious need for some utility within the notes/ news/mail system to modify site tables - on command. Thus when a site changes its name, or a new site is added, or becomes defunct, or changes linkage, someone at the site can send an automatic signal around that gets distributed (as for notes/news) to update the site table at each site.
