In SFL V9.53, Jerry Bakin (Bakin @ HI-MULTICS) quotes one
of Arthur Clarke's characters as observing "once is an
accident, twice is a coincidence, three times is a conspiracy."

If I remember correctly, the first use of that phrase was
from one of the James Bond novels (Goldfinger?):  "once
is happistance, twice is an accident, three times is
enemy action."
