Can anyone help me with the following three questions?  I've been working on
a short reference sheet of episode names, companions, etc., and I haven't
been able to figure these out.

Please reply via mail; if there is enough interest in the answers, (or the
reference sheet, for that matter), I'll be happy to post them.  Thanks.

1.  Did Liz Shaw leave during "Inferno" or "Terror of the Autons"?

2.  What are the BBC id codes for "The Five Doctors", "Planet of Fire", and
    "The Twin Dilemma"?  My best guesses are 6Q, 6R, and 6T respectively.

3.  When during season 20 did "The Five Doctors" appear?  Between
    "Enlightenment" and "The King's Demons"?
