Two more relics of the past, smf(1) and mksmdiag(1), have departed.
Smf was a standalone SMC11 formatter that we never used anyway;
mksmdiag made SMC11 diagnostic tapes (since we'd had some trouble
with unreadable diagnostic tapes).
