The reason I heard that Mary Tamm left was that she was pregnant (try 
working that into the story!). Since Tom Baker and Lalla Ward had become
rather enamored with one another Baker insisted that Lalla Ward continue
as Romana's regeneration and the producers had to agree - they just barely
got away with Romana regeneration and couldn't very well have the Doctor
regenerate without some explanation.
