I made several changes to the terminal tables for the Sanders:
	- all of the tables now have the improved underlining code (I had
	  forgotten to include this in the "format" tables)
	- all of the turn off underlining in the initialization sequence
	- the ten pitch tables both #include a codetab file
	- the vertical motion for uppercase E-acute has been fixed (we caught
	  the mistake for lower case earlier)
