Does anyone know of a good forms management package ?

Some desirable features that come to mind are:
	o unlimited field size (`countably infinite' length, as
	  they say)
	o multiline and variable length fields,
	o constraint checking for baseline types
	o conditional field prompting (ie, if field a is not
	  blank then ask for field b else ask for c),
	o possible inclusion of unspecified text with a field,
	o formatted output reporting,
	o and maybe integration with an editor (vi like ?).
