It is the percent chance of any spell failing to effect the monster. This includes magically-created damage-producing substances such as Fireballs and Cloudkills. The given magic resistance is the chance if the spell was cast by an eleventh level magic user; it is adjusted up or down with caster's level as explained in the MM. Only the resistant creature benefits; it is possible to Fireball a bunch of orcs and fail to singe a demon standing in their midst. That is made clear in Deities and Demigods.
